School & Sports Physicals are taken by appointment & walk in.

For the uninsured the school physical is $ 50.00 - which includes the blue and yellow forms. If immunizations are required, please bring records. Shots for the uninsured are $10.00 each.

Notice 1. Many urgent care centers offer lower rates for school physicals as do some pharmacies. Please inquire- many of these places charge for the forms as a separate fee and most charge full cost for immunizations and do not participate in the Vaccines For Children Program that provides vaccines to the uninsured at a low cost.

Notice 2. Please bring a copy or request by fax the immunization record when you are here for your appointment. Some physician offices charge a fee to the patient for records- this does not apply if the request is made by fax from one doctor's office to another.

Notice 3. If you are moving here from out of state. Florida requires that a physical examination be done and filled out on Florida State Forms. This also applies to the vaccination record. Unless it is necessary, it is usually more economical for families to have the new physical done in Florida not prior to moving here. That way you do not have to pay for two school physicals.

Florida Shots

We enter your child's immunization records into the Florida Shots system so that it may be available to you from your home computer and other physicians/hospitals.
